A gym membership can be medically necessary for a range of common, diagnosed conditions — including obesity, overweight, high blood pressure, prediabetes or type-2 diabetes, high cholesterol, joint pain or arthritis, chronic back pain, depression, or sleep disturbance. The physician evaluates each case, and a letter is issued only when it is clinically appropriate.

Your Letter of Medical Necessity carries a board-certified physician's signature, NPI, state license, and a QR verification code, and it's written to meet the IRC §213(d) standard FSA and HSA plans require. Your plan administrator can verify it at medslip.co/verify or by scanning the QR code, and we include a step-by-step submission guide. As with any documentation, final reimbursement is determined by your plan administrator.
