Terms of Service

MedSlip is a brand operated by SickSlip, Inc., a Delaware corporation. These Terms of Service are a legally binding agreement between you and SickSlip, Inc. Clinical review and issuance of documentation are provided exclusively through SickSlip P.A., a Florida professional association.

1. Nature and Scope of Services

MedSlip is a service of SickSlip, Inc.; the services described in these Terms are provided by SickSlip, Inc. (technology and administrative services) and SickSlip P.A. (clinical review and medical documentation). MedSlip operates a documentation-focused digital platform that facilitates licensed-physician review of your health information to determine whether a Letter of Medical Necessity is clinically appropriate and, when it is, to issue and sign that letter. MedSlip: • Provides documentation (a Letter of Medical Necessity) — it is not ongoing medical care • Does not provide treatment or prescribe medications • Does not replace your primary care provider or an in-person evaluation where one is needed • Does not provide emergency services If a clinician–patient relationship is formed, it exists solely between you and SickSlip P.A., not SickSlip, Inc. If you are experiencing a medical emergency, call 911.

2. Eligibility

You must: • Be 18 years of age or older, OR a parent or legal guardian submitting on behalf of a minor • Be physically located in a U.S. state where SickSlip operates at the time of submission • Provide accurate and truthful information Submission of false or misleading information may result in account suspension or permanent termination.

3. Telehealth Consent

Clinical documentation may be issued via asynchronous telehealth review. Telehealth consent is governed by a separate Telehealth Informed Consent document. By using the Service, you acknowledge: • You have reviewed the Telehealth Informed Consent • You understand the limitations of asynchronous review • You may withdraw consent prior to issuance of documentation

4. HIPAA and Privacy

HIPAA applicability depends on the nature of the services provided and the role of each entity. If SickSlip P.A. is acting as a HIPAA-covered entity and your information constitutes Protected Health Information ("PHI"), the Notice of Privacy Practices governs the use and disclosure of PHI. For information that is not PHI, or where HIPAA does not apply, the SickSlip Privacy Policy governs. SickSlip operates on a cash-pay basis and does not bill insurance.

5. AI Disclosure

The SickSlip platform may use artificial intelligence tools to: • Structure intake information • Categorize submissions • Detect potential fraud patterns • Support workflow routing AI tools do not independently diagnose, treat, or prescribe. All documentation decisions are reviewed and approved by a licensed clinician prior to issuance.

6. Fees and Payment

All services are cash-pay only. Letter of Medical Necessity: $40. Optional rush processing (priority physician review): an additional $6.99 ($46.99 total). Prices may be changed at any time. Payment is processed through third-party payment processors. A Letter of Medical Necessity is valid for twelve (12) months from its date of issue and may be renewed annually while the qualifying condition persists.

7. Refund Policy

Refund eligibility: Automatic Refund • You request a refund before a physician has reviewed your case • A technical failure prevents delivery of your letter Money-Back Guarantee • If your FSA or HSA plan administrator declines your Letter of Medical Necessity, send their written denial to support@medslip.co within 30 days and we will refund you in full No Refund • A letter was issued consistent with the information you submitted (whether a plan administrator ultimately reimburses is their decision — that scenario is covered by the Money-Back Guarantee above, not an automatic refund) • You provided inaccurate or incomplete information • Fraudulent use, or intent to deceive a plan administrator, employer, or the IRS Discretionary Refund • Platform error • Clinician clerical error Corrections & Reissue • If a letter contains a clerical error, we will correct and reissue it at no charge Refund requests must be submitted within 30 days of purchase to support@medslip.co.

8. Limitation of Liability

To the maximum extent permitted by law: SickSlip's total liability shall not exceed the greater of: (a) The amount paid by you in the prior six months (b) $100 We are not liable for: • A plan administrator's decision to accept, deny, or reimburse a Letter of Medical Necessity • Any tax outcome or IRS determination, including denial of a deduction or reimbursement • Indirect, incidental, or consequential damages

9. Trademarks and Third-Party References

MedSlip is a service of SickSlip, Inc., an independent telehealth medical practice. We are not affiliated with, endorsed by, or sponsored by any FSA or HSA plan administrator, employer, or health plan referenced on our website or in our services. References to specific plan administrators (e.g., Optum, WEX, HealthEquity, FSAFEDS, HSA Bank, Lively, Fidelity) are descriptive only — used to identify where a Letter of Medical Necessity may be submitted for reimbursement. Such references are nominative fair use and do not imply any partnership, endorsement, certification, or guarantee of reimbursement. Reimbursement decisions are made solely by your plan administrator. All trademarks referenced are the property of their respective owners. If you believe MedSlip has used your trademark in a way that exceeds nominative fair use, contact support@medslip.co.

10. Governing Law

These Terms are governed by the laws of the State of Delaware.
